Health advocate is available at no cost to employees enrolled in a Cotton Patch Cafe health plan. Health Advocate provides assistance with support medical issues, answering questions regarding diagnosis and treatment, researching the latest treatment options, helping to find doctors, making appointments, researching and arranging second opinions, facilitating pre-authorizations, coordinating benefits, resolving insurance claims/ billing issues, and explaining benefits and sharing of costs.

W-2 Information
- Requests for previous copies of W-2’s should be directed to Payroll. Examples of previous copies: 2018, 2019, 2020, etc. Email: payroll@cottonpatch.com and provide: full name, work location and W-2’s needed, request for previous years W-2’s will be subject to a verification process.
