Health advocate is available at no cost to employees enrolled in a Cotton Patch Cafe health plan. Health Advocate provides assistance with support medical issues, answering questions regarding diagnosis and treatment, researching the latest treatment options, helping to find doctors, making appointments, researching and arranging second opinions, facilitating pre-authorizations, coordinating benefits, resolving insurance claims/ billing issues, and explaining benefits and sharing of costs.

W-2 Information

  • W-2’s will be mailed out no later than January 31st and will be sent to the employee address on file.
  • Address changes completed after December 31st may not be reflected on the employee’s W2 for the following year.
  • It is the employee’s responsibility to let a manager know if their address needs to be updated.
  • Requests for previous copies of W-2’s should be directed to Payroll. Examples of previous copies: 2018, 2019, 2020, etc. Email: payroll@cottonpatch.com and provide: full name, work location and W-2’s needed, request for previous years W-2’s will be subject to a verification process.
